Appointment Types


MSWord: You must have Microsoft Word installed on your computer in order to use this format. Forms can be "filled in" after downloading them to your computer. You *can save* MSWord forms and your work. PDF Files: Click on the "PDF" link if you prefer to simply print the form with Acrobat Reader. A PDF file can only be downloaded and read with Acrobat Reader. Some PDF files are able to be "filled in", but once you have filled in the PDF form you *cannot save* your work. These types of files are read-only and published here for some browsers and platforms.
